JOB DESCRIPTION

 

JOB TITLE:             CASE MANAGER

DIVISION:               SUPPORTIVE SERVICES

FLSA:                     NON-EXEMPT

LOCATION:            BRONX ,NY

REPORTS TO:       SERVICE COORDINATOR

SALARY:               $40,000 - $45,000

 

SUMMARY: Directly provides case management, counseling, life skills coaching and crisis intervention for mentally

ill adults at two separate sites. Proof of COVID-19 vaccinations required.

DUTIES & RESPONSIBILITIES:

  •  Responsible for individual caseload of single adult participants.

  •  Provides supportive counseling

  •  Assists participants in acquiring and incorporating life skills in areas including but not limited to:

  • budgeting, housekeeping, nutrition, time & stress management, community integration and interpersonal

  • skills.

  •  Manages intake and develops a comprehensive assessment for all new parti

  • Develop with each participant a mutually agreed-upon individual service plan reflecting goals identified

  • during the assessment.

  • Meets regularly with participants to update service plans, while assisting them in obtaining needed

  • services/benefits.

  • Communicates with all collateral agencies to ensure participants compliance with service plan.

  • Helps participants obtain benefits including food stamps, on-going financial and housing needs, and

  • health care

  • Responsible for processing move-ins/move-outs.

  • Responsible for issuing all warning letters and other disciplinary reports.

  •  Documents case management activities in accordance with agency and funder guidelines and procedures.

  • Collects service data and participates in program evaluation and quality assurance activities.

  •  Identifies program needs and makes recommendations.

  •  Organizes and facilitates tenant meetings, workshops and special events for participants.

  • Provides crisis intervention and stabilization as needed. May require some after hour’s follow-up to

  • emergency situations.

  •  Performs related tasks as assigned.

     

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

BA or 5 years relevant experience required. Previous experience working with substance abuse, mental health and homeless populations. Valid Driver’s License required. Requires afternoon, some evening and some weekend hours. Strong verbal and written communication skills necessary. Word and Excel proficiency a must.
