State of Massachusetts Community Support Line Specialist in Boston, Massachusetts

The Bureau of Family and Community Health, part of the Massachusetts Department of Public Health (DPH), is seeking a dynamicHuman Services Coordinator IIto make a profound impact on the lives of children and youth with special health needs (CYSHN) and their families. This role is essential in providing high-quality, family-centered technical assistance, referrals, parent education, and support. You will collaborate with a diverse range of public and private agencies to ensure comprehensive support, inclusivity, and cultural competence in service delivery. Your expertise will be crucial in facilitating connections to educational, residential, financial, and clinical resources, ultimately enhancing the quality of care and support for CYSHN. As a Human Services Coordinator, you will conduct thorough clinical assessments, provide detailed consultations, and assist families in navigating complex public and private health insurance issues. This role involves maintaining meticulous records, participating in outreach and training initiatives, and working closely with a variety of stakeholders to promote best practices and innovative solutions in the field. Your dedication will help bridge gaps in services, ensuring that every child with special health needs receives the care and resources they deserve. /_Duties and Responsibilities (these duties are a general summary and not all inclusive):_/ * Provide comprehensive technical assistance, consultation, and training to families and professionals regarding public resources, entitlements, eligibility, and policies impacting CYSHN. * Conduct detailed clinical assessments for eligibility for DPH Care Coordination and other CYSHN Division Programs, analyzing complex cases and making referrals based on guidelines. * Collaborate with families and primary care providers to identify comprehensive service needs for children with special health care needs. * Facilitate client referrals to educational, residential, financial, and clinical providers and services. * Assist families and clinicians in resolving a wide range of public and private health insurance issues. * Consult with state agencies, service providers, parents’ organizations, private vendors, and clinical professionals to coordinate service delivery and participate in multidisciplinary and multi-agency teams. * Aid families in accessing funds for medical and service costs related to children’s special health care needs, such as the Catastrophic Illness in Children Relief Fund. * Collect, maintain, and distribute printed, electronic, and multimedia resources on services and supports for CYSHN, ensuring complete and accurate case records with adherence to privacy policies. * Compile, examine, and report trends and case-related data to facilitate evaluation and quality improvement in the program and department. * Collaborate with supervisors to provide program outreach, training, and development activities, and integrate parent input in Division programs for CYSHN, while administering specific funds such as the Hearing Aid Fund for Children. /_Required Qualifications:_/ * Knowledge of developmental disorders, developmental disabilities, and mental health. * Knowledge of community resource systems and billing systems. * Understanding of basic clinical skills, human behavior, social work, health care, and law. * Familiarity with medical terminology. * Proficiency in basic computer skills, including word processing and email. * Effective communication skills. * Ability to negotiate, mediate, and diffuse conflict. * Capacity to think quickly and make independent judgments. * Professional representation of the department. * Ability to plan, assign, and coordinate activities, along with team-building skills. /_Preferred Qualifications:_/ * Bilingual in languages such as Haitian Creole, Portuguese, Vietnamese, Arabic, Spanish and English preferred. * Experience in primary health care and community health social work is an asset. /_DPH Mission Statement:_/ The mission of the Massachusetts Department of Public Health (DPH) is to prevent illness, injury, and premature death, to assure access to high-quality public health and health care services, and to promote wellness and health equity for all people in the Commonwealth.
