The Stroke Nurse Navigator is responsible for the navigation of stroke patients seeking treatment at St. Elizabeth's Boardman Hospital and Saint Joe's Warren for the the facilitation of care through the assessment, treatment, discharge, and continuum of outpatient follow up services. Serves as the liaison between the patient and caregivers as indicated by the disease specific team. Coordinates appropriate multidisciplinary communication. Assists Neuroscience Director/ Coordinator in duties as assigned to support the Disease Specific Certified population service area. Leads Disease Specific Joint Commission Certifications as a member of the team. Develops and becomes content expert of certification standards from joint commission for the proliferation and stewardship of stroke program.

Performs duties to provide the highest level of patient care and satisfaction. This role encompasses practice and education for the navigation of the patient and their family for the duration of their hospitalization and post discharge phase.

ESSENTIAL FUNCTIONS

40% Focus/Scope include:

Oversight of the interdiscipinary care for acute ischemic stroke, TIA, non traumatic ICH and SAH patients.

Assesses the patient's plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs.

20% Provide daily real time feedback and education for staff in addition to performing both concurrent and retrospective chart review, data entry and data management of all stroke patients admitted to St. Elizabeth Youngstown Designated Stroke Center.

5% Participate in the development of educational materials and education of all non clinical and clinical (rehabilitation medicine, nutritution, social work, care management) staff, stroke patients and their caregivers.

5% Serves as a resource to nurse managers in the identification of staff learning/performance needs Works with physicians to bridge educational needs and programs for implementation of quality metrics.

10% Provide expert nursing care consultation and bedside support and practice oversight providing expertise in comprehensive stroke treatment in the emergency departments, and intensive care unit.

10% Attends stroke alerts when able to ensure timely delivery of treatments and identify any potential process breakdowns.

10%Data collection and analysis to initiate changes as appropriate to improve patient outcomes.

MINIMUM EDUCATION

Required Minimum Education: Bachelor's degree in Nursing or ADN with BSN in progress

Preferred Education: Specialty/Major MSN, MBA, MHA

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Required:BLS Certification

Preferred: ACLS Certification

MINIMUM QUALIFICATIONS

Minimum Years and Type of Experience: 3 years in nursing practice with focus on clinical care of stroke patients. Critical care experience preferred.

Other Knowledge, Skills and Abilities Required: Superior interpersonal, organizational and communication skills

Other Knowledge, Skills and Abilities Preferred: Knowledge of evidence based neurology and neurosurgery care focused on ischemic and hemorrhagic stroke care.
