HealthPartners Pharmacist, Medication Therapy Management in Bloomington, Minnesota
HealthPartners is hiring an MTM Pharmacist. This role provides clinical pharmacy services to the HealthPartners health plan's members and to HealthPartners Medical Group and Clinics patients. They provide support to patients, their families, and physicians in addressing medical and drug-related concerns; educate and empower patients and families to make informed personal health care decisions; and facilitate communication between patients, physicians, and other health care providers. This role supports pharmacy quality and utilization improvement programs and clinical pharmacy services, and to provide specialized clinical pharmacy services, education, clinical research, and related duties as assigned. REQUIRED: Must be licensed as a Pharmacist in the state of Minnesota. Must maintain this license in good standing as required by Minnesota law. Must be licensed as a Pharmacist in the state of Wisconsin within six months of hire. Must maintain this license in good standing as required by Wisconsin law. Graduated from an accredited college of pharmacy on or after May 1996, or completed a structured and comprehensive Medication Therapy Management (MTM) education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire. Two years of experience providing MTM services as part of a multidisciplinary team within the last five years, or successful completion of a one year post-doctoral residency focused on providing MTM/ambulatory care pharmacy services. Working knowledge of pharmacology, pharmacodynamics, and pharmacokinetics, pathophysiology and therapeutic disease states commonly found in ambulatory patients. Strong leadership and organizational skills. Maturity and dependability are essential. Ability to organize and function under stress with many interruptions. Ability to establish and maintain effective, friendly and courteous working relationships with coworkers, physicians and patients. Strong communication and listening skills. Ability to accept and implement change. Ability to travel to multiple work locations. PREFERRED: Pharm.D. Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist Experience with Microsoft Suite and pharmacy related applications such as Epic ACCOUNTABILITIES: Provide Medication Therapy Management services to health plan members and HealthPartners Medical Group Clinics' patients. Assure that all of a referred patient's drug therapy is used appropriately for each medical condition, the most cost-effective drug therapy available is used, the safest drug therapy possible is used, and the patient is able and willing to take the medication as intended. Address each patient's understanding of, expectations, and concerns with their drug therapy. Consult with patients regarding physical findings relevant to monitoring drug therapy. Formulate patients' treatment plans and initiate, modify, and continue appropriate drug therapy as outlined in approved treatment protocols under the Scope of Practice privileges or in consultation with the primary physicians. Initiate requests for appropriate laboratory and other diagnostic studies as needed for monitoring drug therapy. Provide follow-ups by phone, letter, or appointment with patients. Analyze laboratory and diagnostic test data to modify drug therapy and dosing as necessary. Appropriately refer primary care patients to other health care practitioners, including nutritionists, nurses, social workers, etc. Implement protocols approved by the Pharmacy and Therapeutics Committee regarding the use of formulary, nonformulary, and restricted drugs. Provide verbal and written education and training designed to enhance every patient's understanding regarding the appropriate use of patient's medications. Act as a pharmacy liaison for the plan and HealthPartners contracted medical clinics. Conduct or assist in conducting DUR/DUE studies. Responsible for the direction of and participates in the education of pharmacy students and postdoctoral residents. Participate in applied clinical pharmacy research. Prepare and present new drug review and therapeutic class reviews for the HPMG clinics. Provide pharmacy academic detailing activities to HealthPartners contracted medical clinics to reinforce plan's initiatives and formulary. Provide effective drug information and consultation to professional staff, providers, patients and members. Maintain a professional image. Adhere to the established Group Health, Inc., patient-service standards. Maintain records and reports documenting all contacts with patients in accordance with regulatory, departmental and professional standards. Support and cooperate with other department's operation initiatives. Perform other duties as assigned by the Clinical Pharmacy Program Manager.
