We are seeking a community-based, Registered Nurse Navigator to join our team in our Bethlehem, PA location. The primary role of the Nurse Navigator is to: function as a health advocate to aid individuals in receiving the physical health and behavioral health care assist in coordinating medical and behavioral health appointments, hospital care and discharge to the community arranging travel to appointments development of personal plans for consumer health collaboration and wellness. Requirements
The ideal candidate will possess the following qualifications: Licensed as a Registered Nurse in Pennsylvania Currently meets all requirements per the state nursing board Minimum one year experience working with individuals who have mental health diagnoses Experience with addictive diseases and/or community mental health preferred
