Job Title: Physical Therapist
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The successful candidate will evaluate patients, establish goals, and develop individualized treatment plans. This role involves providing direct patient treatment based on current physical therapy practices and revising treatment plans based on the patient's response. Additionally, the Physical Therapist will educate patients and their families regarding patient abilities and the need for assistance and continued therapeutic intervention. This position requires consultation with physicians on patient evaluations, progress, and complications with treatment, as well as maintaining medical and departmental records according to department policies and procedures.
Responsibilities
Evaluate patients and establish goals for treatment.
Develop individualized treatment plans.
Provide direct patient treatment based on current physical therapy practices.
Revise treatment plans based on patient's response.
Educate patients and families regarding patient abilities and need for assistance.
Consult with physicians on patient evaluations, progress, and complications with treatment.
Maintain medical and departmental records according to department policies and procedures.
Participate in patient care conferences, department meetings, and hospital in-services.
Communicate with other professional staff and patient's family/support system to provide continuity of care.
Essential Skills
Experience with EMR systems
Inpatient/Outpatient care
IL PT License
Doctor of Physical Therapy or Bachelor's degree in Physical Therapy
CPR Certification
Ability to communicate effectively with staff at all organization levels
Effective written and verbal communication skills
Strong customer service skills
Additional Skills & Qualifications
Open to Entry Level/New Grads
Experience with Meditech EMR systems is a plus
Experience in an outpatient setting preferred, but not required
Bilingual in English/Spanish is a plus, but not required
Pay and Benefits
The pay range for this position is $80000.00 - $113000.00
This full-time outpatient position offers flexible days and hours, time off and reimbursement for CEUs, licensure renewal, and mentorship for new graduatesHealth Insurance, PTO, 401k, Dental, Vision

This is a fully onsite position in Berwyn,IL.
