Position Summary

The Case Management Services Director of Texas Family Care Network will oversee and manage the case management department, all its functions and ensure the effective delivery of services to clients. This position will lead a team of Case Management Supervisors, providing guidance and support to ensure efficient operations and excellent client care. This role requires strong leadership, and interpersonal skills to effectively oversee case management functions, meet outcomes and collaborate with internal and external stakeholders. Overall, as the Director of Case Management, your role is crucial in providing strategic direction, ensuring high-quality services, and advocating for clients.

Essential Roles and Responsibilities

  • Leadership and Management:

  • Provide strategic direction and leadership for the case management department.

  • Develop and implement policies, procedures, and protocols for effective case management services.

  • Supervise and mentor case managers and support staff, ensuring their professional development and performance.

  • Foster a positive and inclusive work environment that promotes collaboration and teamwork.

  • Client Assessment and Advocacy:

  • Collaborate with case managers to develop individualized care plans and goals for clients.

  • Advocate for clients' rights and needs, ensuring they have access to necessary resources and support systems.

  • Address any client concerns or complaints and implement appropriate resolution strategies.

  • Collaboration and Networking:

  • Establish and maintain collaborative relationships with external stakeholders, such as government agencies, community organizations, and healthcare providers.

  • Participate in relevant committees, task forces, and community meetings to represent the organization and advocate for social services.

  • Data Management and Reporting:

  • Ensure accurate and timely documentation of client information, case progress, and outcomes.

  • Generate regular reports on program activities, achievements, and challenges for management and funding agencies.

  • Utilize data analysis to identify trends, measure program effectiveness, and make informed decisions.

  • Compliance and Quality Assurance:

  • Stay updated on relevant laws, regulations, and ethical guidelines pertaining to case management and social services.

  • Ensure compliance with legal and regulatory requirements, including privacy and confidentiality standards.

  • Develop and implement quality assurance measures to monitor and enhance the quality of case management services.

  • Professional Development:

  • Stay informed about best practices, research, and advancements in the field of social services and case management.

  • Attend conferences, seminars, and training opportunities to enhance your knowledge and skills.

  • Encourage and support the professional development of case management staff.

Essential Qualifications

  • Location: Must reside in Region 5

  • Education/Credentials/Licensure: Bachelor's degree in Social Work or related field required; Valid driver’s license required

  • Experience: 2 years in a Supervisory position; 3 years in Case Management of Child Welfare

  • Clearances: State Police/FBI per state regulations; Child Protection

Working Conditions

  • Physical Demands: Requires vision, speech, and hearing and a moderate level of physical activity including: sitting, standing, walking lifting and carrying as required.
