Pay

$42 (new grad) to $49 (depending on experience) an hour

Shift

• 40 hours a week

• 8 - 4 or 9 - 5

• Make your own schedule depending on the patient's availability

Job Details

• Mileage reimbursement $0.60/mile.

• Perform comprehensive evaluations of clients in their homes, focusing on aspects such as mobility, strength, flexibility, and overall physical function.

• Create customized treatment plans derived from home assessments, taking into account each client's unique needs and living conditions.

• Administer various therapeutic interventions, exercises, and modalities to enhance clients' mobility, strength, and functional abilities in their home environment.

• Oversee Physical Therapist Assistants by conducting case conferences, co-signing treatment notes, and performing on-site supervisory visits.

• Educate clients about their conditions and treatment plans, offering clear instructions on home exercises to support ongoing progress.

• Regularly reassess clients and adjust treatment plans as necessary to ensure optimal results in a home-based setting.

• Apply effective pain management techniques to improve clients' overall well-being.

• Maintain clear communication with clients, their families, and other healthcare team members to deliver tailored and effective home-based care.

Qualifications

• Required: Active NJ PT license.

• Required: Experience working with geriatric patients.

• Required: Have an active driver's license and have a vehicle with insurance.

• Preferred: At least 2 years experience providing care in patients' homes, however, new grads are welcome to apply.
