NOTICE OF POSITION OPENING

POSITION:   Treatment Court Clinical Case Manager

DEPARTMENT:   Center for Counseling & Psychological Services (CCPS)

DIRECTLY RESPONSIBLE TO:   CSIP Project Coordinator

SUMMARY OF POSITION:   The Treatment Court (TC) Clinical Case Manager will provide support to individuals connected to the Washington County Treatment Court who have been identified for case management supports/services for substance use disorders. The TC Clinical Case Manager will be responsible for working in collaboration with the Treatment Court Team, act as a liaison between the agency and the Treatment Court administration and will work to connect caseload to community-based resources necessary to support positive treatment outcomes.

SPECIFIC DUTIES AND RESPONSIBILITIES:

 

  • Providing support services to clients which will include but is not limited to:

  • outreach visits,

  • supportive counseling,

  • crisis interventions,

  • assistance with independent living skills,

  • transportation of clients in personal vehicle when clinically indicated.

  • Providing support to secure safe/appropriate housing and assist with employment search.

  • Empowering clients to be more responsible for their own lives by collaborating with them in developing their individual plan of care, teaching them the skills they need to live in the community and other psychoeducation as indicated.

  • Collaborating and consulting with Treatment Court Team members and Community Service providers.

  • Providing on-going assessments and treatment planning including coordination of treatment team meetings as needed.

  • Attending required meetings and trainings, including individual and/or group supervision and other agency committee work.

  • Completing all required paperwork on time to include agency documentation as well as producing reports as required for grant administration.

  • Fulfilling other duties appropriate to your position as assigned.

    QUALIFICATIONS:  

  • Bachelor’s degree in human services or criminal justice required, master’s degree preferred.

  • Minimum of 2 years of experience working in human services, a focus on substance use preferred.

  • AAP certification or willingness to obtain AAP certification within 180 days of hire required.

  • Able to move from a seated position to a standing position with or without accommodation.

  • Able to drive clients in own personal vehicle to and from community-based events without accommodation.

  • Access to a safe, reliable, and insured vehicle.
