The Collaborative: A Healing Centered Community at University of Maryland, Baltimore has an exciting opportunity for a dedicated, caring, and team-oriented Contingent Category II Community Outreach Worker I/Community Outreach Worker II. The selected candidate will apply specialized and theoretical knowledge, principles, and concepts to conduct outreach in the community to inform stakeholders of case management services. The candidate will enroll clients into case management services and provide case management. This position will report to the Community Program Director. The mission of The Collaborative is to prevent, intervene, and heal from trauma and violence in all forms. Contingent Category II employees receive a generous leave package that includes 10 vacation days, 9 holidays, 5 sick days, 3 personal days and comprehensive health insurance; professional learning and development programs; Contingent Category II employees are eligible for tuition remission for the employee only and the tuition remission only applies to courses taken at UMB.  Tuition remission for Contingent Category II employees who are part-time will be proratedin accordance withtheir FTE; and flexible work schedules and teleworking options (if applicable per job).    PRIMARY DUTIES (CommunityOutreach Worker I) * Recruits a client base within the community byidentifying,locating, interviewing, and screening individuals who may beappropriate forthe program.  * Provides general information to individuals and families on programobjectivesandservices;eligibility requirements andbenefits;confidentiality of information etc. Distributes informational materials and literature.  * Conducts visual inspection of the physical condition of theclient`shouse toidentifyfactors that may be detrimental tomaintaininga safe, healthy, and comfortable living environment.  * Serves as a liaison between the client and community resources including department staff, City, State, and Federal social services agencies.  * Schedules clients for appointments with health care providers, reminds them of pending appointments, and contacts them to inquire into reasons for missed appointments. Escorts clients to various appointments to ensure compliance and provide support.  * Assistswith client retention by following up on all contacts for continued progress assessment andlocatingthose clients who have moved or lost contact with the program.  * Assists with the facilitation of the clinic process by greeting clients and making sure that they are registered to be seen.  * Assistswith planning, organizing, and implementing communityspecial eventssuch as health fairs, workshops etc.  * Participates in staffmeetingand conferences. Reports on community outreach activities including problems and concerns.  * Prepares written reports andmaintainsrecords of outreach contacts and activities.  * Updates information in database from various data forms collected.  * Maintains a good working relationship with all members of the community.  (Community Outreach Worker II) * Provides crisis intervention through individual counseling, providing basic health education,assistingclients access various social services, and referring clients to alternate care facilities. * Conducts initial assessment in order to determine individual client needs. * Speaks before neighborhood groups and attends community meetings toestablishcommunication and rapport; to promote services; andassistin resolving problems facing the community. Prepares and distributes informational literature. * Participates in the evaluation of materials and methods designed to meet the educational needs of the client community. * May provide guidance and training over other outreach workers. * Performsall ofthe duties of a Community Outreach Worker I. EXPERIENCE:None OTHER:Except for qualificationsestablishedby law,additionalrelatedexperienceand formal education in which one has gained the knowledge, skills, and abilitiesrequiredfor full performance of the work of the job class may be substituted for the education or experience requirement on a year-for-year basis with 30 college credits being equivalent to one year of experience. (Community Outreach Worker II) EDUCATION:High-school diploma or GED. EXPERIENCE:Two years in community outreach, health care, research, and/or social services. OTHER:Except for qualificationsestablishedby law,additionalrelatedexperienceand formal education in which one has gained the knowledge, skills, and abilitiesrequiredfor full performance of the work of the job class may be substituted for the education or experience requirement on a year-for-year basis with 30 college credits being equivalent to one year of experience. REQUIRED KNOWLEDGE/SKILLS/ABILITIES:  Ability to communicate effectively orally and in writing. Ability toestablishand maintain effective working relationships. Ability tooperatecomputers and other office equipment. Ability towritereports andmaintainrecords. Ability to work in extreme weather conditions. Ability to perform extensive standing and walking. Ability tohandle sensitiveand confidential matters with discretion and tact. Abilityto interact with the public, health care providers, community organizations, and social service agencies. Ability tomaintainpoise and courtesy under pressure.  CONDITIONS OF EMPLOYMENT:  Employees in this job class maybe requiredto use their personal automobile while conducting official business andpossessa valid Maryland Non-commercial Class C or Commercial Class B Driver's License (CDL). Candidates selected for employment may be subjected to medical inquiries and/or medical examination todetermineabilitytoperform thejob. Employees in this job class maybe requiredto successfully pass a police background check.  Based on the duties of this position, selected candidates/are not/eligible to participate in the Bargaining Unit (Union). Salary Range for theCommunity Outreach Worker I: $34,613 - $38,500, commensurate with education and experience.
