Bon Secours Mercy Health Case Manager - Returning Citizens Program in Baltimore, Maryland

The Case Manager will manage a caseload of program participants that enrolled in Bon Secours Community Works’ Returning Citizens Program. The position will ensure that all clients have a positive training outcome by ensuring the effective implementation of recruitment strategies, enrollment, education, case management, and reporting. The Case Manager will work with clients in a one on one and a group setting with the goal of reducing barriers and placing them into careers.

  • Oversee the management of Bon Secours Community Works’ Returning Citizens’ Program

  • Refine all aspects of program design and document it in a standard operating procedure.

  • Responsible for data management and performance reporting.

  • Monitors data quality, collects monthly data reports, and submits reports to the Program Director on a specified basis.

  • Maintains all records and outcomes in Apricot and/or additional databases.

  • Refine and shift program delivery strategies based on data analytics.

  • Work collaboratively with the Program Manager to improve and expand the Returning Citizens Program.

  • Works collaboratively with the Program Manager to monitor progress towards meetings grant performance metrics.

  • Works collaboratively with the Employment Services Coordinator to develop strategies for eliminating barriers and increases employability of clients.

  • Maintain and develop key partnerships that increase client outcomes.

  • Participate in outreach, recruitment, and engagement activities in the community.

  • Facilitate client development groups using evidence-based manuals and curriculums.

  • Remain informed of evidence-based practices and makes changes to programming when relevant.

  • Deliver presentations and participate in conferences that communicate program strategies and outcomes.

Employment Qualifications

Required Minimum Education:

Bachelor’s Degree, Specialty/Major, Psychology, Social Work, Business, Human Services, Human Resources, Education or equivalent from an accredited college or university or equivalent experience is required

Preferred Education:

Masters Degree, Business, finance, social work, public health, human services, education

Minimum Years and Type of Experience:

1 year of professional experience in working with clients who are: incarcerated, on parole/ probation, on home confinement, or recently released from home confinement.

Two years of supervising program staff and managing programs.

Other Knowledge, Skills and Abilities Required:

Organizational skills, record keeping skills, and time management skills.

Other Knowledge, Skills and Abilities Preferred:

Proficiency in oral presentations, writing skills, and Microsoft Office Suite. Effective verbal, oral presentation, and written communication skills.

Strong interpersonal skills necessary to develop and maintain relationships with diverse stakeholders.

Detail oriented with excellent critical thinking skills

Combination of post-secondary education and experience in lieu of a degree.
