Position Description

Penn Medicine at Home is seeking a per diem Nurse Practitioner to join their team! The Nurse Practitioner will support Penn Hospice programs in the care of seriously ill patients and their families. The role will be responsible for expert clinical practice in symptom management, care coordination, and end-of-life care management for patients receiving hospice services. The per diem Nurse Practitioner will provide coverage around the Philadelphia area with shifts opportunities across days, nights, and weekends, including overnight telephonic support.

Minimum Requirements

Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania, and possess a MSN or DNP and certification as a Family Nurse Practitioner or Adult Gerontology Nurse Practitioner. The ideal candidate will have at least 3 years of NP clinical experience.
