PURPOSE AND SCOPE:
Coordinates the care and follow up for late stage Chronic Kidney Disease (CKD) patients to ensure a planned transition into Renal Replacement Therapy (RRT). Focuses on improving the preparation of patients who need to start RRT, ensuring each patient receives the most suitable permanent access, while minimizing the use of temporary catheters. Ensures every patient receives treatment modality education and dialysis preparation in order to begin RRT at the facility designated by the Nephrology Practice and the patient. Coordinates late stage CKD education, vascular access management support and treatment option initiatives and support processes that improve patient outcomes and reduce patient hospitalizations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Under general supervision, follows established company policies and procedures and applies acquired job skills. Work collaboratively with the Renal Care Program team to carry out the goals and objective of the related FMC Renal Care management program.
Performs functions that require full knowledge of general aspects of the job.
Utilizes software applications or other appropriate electronic health records (EHR), to develop and maintain a HIPPA compliant clinical database of patients with CKD in the program.
Works on assignments that are semi-routine in nature, but recognizes the need for occasional deviation from accepted practice.
Provides general reports and analysis, identifying trends, anomalies and areas of concern to the patient’s providers on a semi-routine basis.
Encourages participation, scheduling, and follow up on members attending educational CKD classes. Works with staff members to ensure member’s participation.
Initiates and participates in the collection and analysis of clinical data that supports the CKD protocol.
Develops processes to educate the nephrologist and their staff on the CKD program and how it can benefit them, their office, and the member.
Obtains nephrologist or PCP treatment plan to assist member through the CKD process, i.e. Scheduling appointments, vein mapping, follow up calls, access; or helping member understand the importance of being an active and educated participant in their treatment plan.
Develops processes with the nephrology office on those members in denial to get necessary labs; access, and to those programs supported by the nephrologist.
Identifies and refers members to other contracting partner programs per their individual needs.
Participates in meetings to review the CKD program with the practice contracting partner and to review pertinent CKD data.
Participates in the annual CKD meeting and other meetings as requested by the CKD administrative team.
May refer to senior level staff for assistance with higher level problems that may arise.
Escalates issues to supervisor for resolution, as deemed necessary.
Review and comply with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations.
Assist with various projects as assigned by direct supervisor.

Day to day work includes desk and personal computer work and telephone interaction with patients, patient families, facility staff, physicians and the community at large. The work environment is characteristic of an office environment with air temperature control and moderate noise levels.
Travel to regional, divisional and corporate meetings may be required.
- High School Diploma required
EXPERIENCE AND REQUIRED SKILLS:
1 – 2 years’ related experience in clinical patient care or case management required.
A combination of hemodialysis, PD, transplantation, CKD education, case management and nutritional training highly valued.
Renal transplant, dialysis, or CKD patient care preferred.
Demonstrated knowledge of renal disease and renal transplant preferred.
Excellent written and verbal communication skills.
Ability to communicate and maintain effective interpersonal relationships at various levels of the organization.
Good understanding of relationship between the dialysis providers and the physician practice.
Ability to determine when coordination may be performed by telephone or written instruction and when approval by a higher level of authority such as a physician or other health care provider is required.
Must be highly self-motivated, dependable and organized with basic computer skills.
