Job Overview
Licensed vocational nurses (LVNs) provide basic medical care, including taking vital signs, explaining procedures to patients, administering medications, and tracking patient progress. LVNs also collect and send samples to medical labs for testing, complete patient paperwork, and collaborate with physicians to determine a care plan for patients. LVNs must report to a registered nurse (RN) or physician. They are an integral part of a patient care team.
Job Responsibilities
- Evaluate the needs of the member, the resources available, and recommend and/or facilitate the plan for the best outcome
- Assists with developing ongoing long-term care plans/service plans and works to identify providers, specialists, and/or community resources needed for long-term care
- Coordinates as appropriate between the member and/or family/caregivers and the care provider team to ensure identified services are accessible to members
- Provides resource support to members and their families/caregivers for various needs (e.g. employment, housing, participant direction, independent living, justice, foster care) based on service assessment and plans
- Monitors care plans/service plans, member status and outcomes, as appropriate, and provides recommendations to care plan/service plan based on identified member needs
- Interacts with long-term care healthcare providers and partners as appropriate to ensure member needs are met Collects, documents, and maintains long-term care member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators
Job Requirements
2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions
Meet required deadlines, 3-day turnaround time for documentation of any assessment.
Deliver thorough assessment, clear documentation.
Available to work M-F 8-5, with possibility of work needed outside of 8-5 schedule.
75% Travel required to complete assessments, possibility of travel of more than 30 miles to reach or members.
