Description of Job

The Dietitian II will facilitate direct care staff in fulfilling the nutritional needs of facility residents. Duties include: accurate assessment, appropriate nutritional intervention, follow up and evaluation of nutritional care; providing nutritional counseling to residents who are able to participate in nutrition decision making; active participation in total care planning with an interdisciplinary team; education and training of new and existing food service operations staff with emphasis on meeting nutritional, quality, sanitation and therapeutic standards; ongoing evaluation of food service operations for quality assurance purposes and to assure compliance with guidelines.

Additionally, as a Dietitian II you can expect to:

  • Collect pertinent data, including: diet history, diagnosis, biochemical data, medications, anthropometric data and assesses the nutritional status of residents develop plans of nutrition care and provides intervention recommendations to meet nutrition needs

  • Provide consistent follow-up and reevaluation of nutrition care process; provides interventions as necessary. Assessments include but are not limited to significant weight changes, nutritionally related weight issues, enteral feedings, dialysis, and any other medical conditions requiring nutritional interventions.

  • Provide nutritional counseling to facility residents and their families, as needed, who are able to actively participate in their own nutrition decision making. Attend Care Conferences and document resident’s needs, preferences, and interventions related to nutritional well-being and when necessary, document when resident chooses not to adhere to recommendation.

  • Continuously evaluate food service operations for quality, sanitation, nutrition adequacy, and adherence to dietary standards through meal observations and audits, and assists in identifying and implementing necessary corrective actions. Participate in operations of total quality improvement for the food service department, such as providing reports when necessary to the facility’s QAPI (Quality Assurance Performance Improvement) team, identifying problems, collecting data, formulating and implementing solutions, and evaluating outcomes.

  • Provide training and education for staff involved in food service operations on preparation and type of foods necessary to meet nutrition, safety, quality, and therapeutic standards. Meet continuing educational requirements necessary for maintaining Dietitian Registration. Provides in-house education during New Employee Orientation, and Dementia in-house Training Class, and other in-services as needed. Educate new dietetic interns to long term care.

  • Re-evaluate and make recommendations concerning facility menus, taking into consideration resident likes and dislikes.

  • Special projects and Other Duties as assigned

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

MINIMUM QUALIFICATIONS:

Experience Only:

Five (5) years of relevant experience in a long term care setting

OR

Education and Experience:

A combination of related education and/or relevant experience in an occupation

related to the work assigned to this position equal to five (5) years

Special Qualification:

  • Active Dietitian Registration

Conditions of Employment:

  • CDHS employees (all Direct contact with vulnerable persons):

CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

  • Must be willing to test for Tuberculosis (TB) test.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.

Sign-On Incentive:

Upon approval, a sign-on incentive based on position, ranging from $2,000 to $7,000 (prorated if a part-time position), will be given in three payments: within two pay cycles of when the new employee starts at CDHS, 180-day anniversary and 270-day anniversary with CDHS. These incentives will be available to the first 1,000 new employees in select high-need positions.
