As an accredited public health department, we're seeking highly qualified candidates for the position of*Communicable Disease Specialist Supervisorto join theNorth Central Health Districtin Macon, GA.*
Job Responsibilities
Plan, organize, coordinate, monitor and ensure implementation of policies, procedures, guidelines, and standards for the communicable disease program.
- Review/monitor all case management activities, services, data collection and entry, analysis and use information to develop and implement care plans.
- Provides technical guidance regarding infectious diseases to all sectors of the medical and public health community.
- Coordinate program operations with other district programs and county health departments to ensure quality and continuity of services to all customers are met.
- Coordinates the training of district and county public health staff, private medical providers, and the general public in public health and infectious disease issues.
- Participates in quality management meetings to collaborate, identify and report on issues such as barriers to care or partner notification.
- Interpret existing laws, regulations, guidelines relating to programmatic procedures/issues toward training/compliance.
Provide direct diagnostic, therapeutic, and case management services for public health patients in both public and field setting.
- Investigates and locates HIV and STD exposed or diagnosed patients and determines if and where they are engaged in care via partner service interviews.
- Ensures implementation of timely disease intervention strategies to include field investigations and home visits to decrease the prevalence of such diseases.
- Provide case management services/activities for patients following policy, procedure, and protocols.
- Evaluates programs and plans using performance management and quality improvement initiatives to ensure compliance with funding requirements and community needs.
- Collaborates with District and State Epidemiology, Clinical, HIV, and STD staff to assist with identifying patients who are not in care or lost to care and are unable to be brought back into care.
- Actively refers and assists with linking HIV-positive individuals to care and assists with re-engaging HIV positive individuals back into c re.
Manage the communicable disease program by supervising and planning work for assigned staff.
- Prepares and submits monthly service reports and quarterly narratives.
- Prepares and completes reports and surveillance activities as required by the District Leadership and DPH partners.
- Works with NCHD staff to assist with marketing intervention to community partners.
- Maintains consistent communication and coordinates services and activities with Communicable Disease Specialists, HIV Linkage Coordinators, and other program staff.
- Maintain knowledge of current trends/developments in the field of infectious-communicable disease and apply pertinent new knowledge to performance of other responsibilities.
- Performs various duties in support of section and district operations and utilize the Program Collaboration and Service Integration evidence-based practice with all staff of the Office of Epidemiology, Community Assessment, and Research initiatives.
- Responsible for updating the Dashboard with assigned Communicable Disease measures in a timely manner.
- Ensure that quality improvement processes are utilized as necessary to ensure evidence-based practices are implemented within the program that assist in reaching Public Health 3.0 goals.
- Participates in monthly district management team, local APIC chapter, monthly/quarterly state program, staff, and monthly Epidemiology Partners meetings.
- Represent District Leadership at meeting, forums, public meeting and media events as requested.
Minimum Qualifications
Bachelor's degree from an accredited college or universityANDTwo years of related experience
ORAssociate's degree from an accredited college or universityANDFour years of experience in a related area
ORSix years of experience in a medical related field
ORLicensure as a registered nurse AND Three years of nursing experience
OROne year of experience at the lower level Communicable Disease Spec 3 (HCP082) or position equivalent.
Preferred Qualifications
In addition to the minimum qualifications preference will be given to applicants with experience with:
- Masters degree in a public health or science related field.
- Providing STD/HIV/AIDS case management services.
- Must have good working knowledge of best practice counseling approaches. interventions and strategies that have been proven effective with the high-risk populations.
- Must have a working knowledge of STD/HIV/AIDS and treatment, partner services, and linkage to care services.
- Must be experienced with a multi-disciplinary team approach.
- Prior networking and community outreach experience desired.
- Department of Public Health (state or district) experience.
