Randolph Hospital Social Work/Case Manager (MSW) in Asheboro, North Carolina
In collaboration with Care Managers, patients/families, physicians, caregivers, staff, and community agencies, the Social Worker coordinates implements and arranges for the post-discharge extended care, home health and durable medical equipment for patients. Consults, collaborates and coordinates plan of care with physicians and providers concerning behavioral health clients. Provides crisis intervention, and emotional support with regard to diagnosis, treatment, and post-discharge plan. Identifies and accesses resources to optimize effective and beneficial patient care outcomes in collaboration with a multi-disciplinary team. Master's Degree from an accredited School of Social Work required. Excellent communication skills required, with an emphasis on oral and listening abilities. Possess analytical thinking, problem solving aptitude and ability to set priorities. Knowledge of treatment modalities and alternate healthcare delivery systems is preferred. Minimum one year experience working in a Behavioral Health organization or with Behavioral Health Clients. Healthcare related experience preferred. Knowledge of social and health care resources within the community. Familiarity with standards for medical social work practice.
