RN Care Transition Manager / ED Patient Navigator / Full Time

Work location: Texas Health Arlington Memorial, 800 W. Randol Mill Road, Arlington TX 76012

Work hours: Full Time, 36 hours weekly, Days, (3) 12hr shifts (Sunday, Monday and Tuesday)

Texas Health Arlington Memorial Hospital has a rich history in North Texas serving the local communities of Arlington, Kennedale, Pantego, Mansfield and Grand Prairie. Here’s What You Need

  • Bachelor's Degree, Bachelor of Science in Nursing (BSN) is required. (CLIAFE) Must comply with CLIA regulations regarding foreign education equivalency required.

  • 3 years of RN experience in an acute care hospital is required.

  • 1 year RN Case Management experience preferably in an Emergency Department setting is preferred.

  • 1 year of discharge planning and/or care management is highly preferred.

  • RN Emergency Department experience is preferred.

  • Must be able to work independently in a case management/discharge planning capacity.

  • Must hold an active RN license - Registered Nurse license Upon Hire is required.

  • CPR – Cardiopulmonary Resuscitation is required upon hire.

  • ACM – Accredited Case Manager upon hire is preferred OR CCM – Commission for Case Manager Certification or ACM – American Case Management Certification upon hire is preferred.

What You Will Do

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner. In addition to the required qualifications, a successful RN will:

  • Serves as a point of contact for all identified stakeholders. Attempts to schedule PCP, specialist or clinic follow up appointments for patients.

  • Promotes discussion and assists in the identification of a primary care physician (PCP) for patients without a PCP.

  • Clinical review staff requirements and communications
