Blair County Community Action Agency SSI Representative Payee Case Manager in Altoona, Pennsylvania

  1. Interview clients to determine eligibility and document case file and perform client assessments to identify strengths and weaknesses with regard to addressing issues and life skills; and develop a case plan for client to establish goals and refer clients to other service providers as needed;

  2. Meet with clients as needed to verify their needs are being address;

  3. Maintain records, keep progress reports and document progress in client case file and in computer database. Provide feedback to clients and revise client goals as needed;

  4. Accurate completion of reports to funding sources and Social Security when requested;

  5. Coordinate services with other internal programs and other organizations in the community;

  6. Attend and successfully complete certification courses, seminars, and meetings as directed by your supervisor(s), the Deputy Executive Director and/or the Executive Director;

  7. Assist other programs/departments within the Agency as needed;

  8. Per signing out, maintain and track mileage for Agency Vehicle.

  9. Participate in the Annual Point in Time Count;

  10. Maintain a case load of 55 clients;

  11. Actively complete and submit distribution slips to Financial Department in a timely manner;

  12. Maintain client ledger, to keep accurate payments updated.

  13. Maintain an open communication with Social Security office, to ensure all reports and documents are completed and submitted in a timely manner. Complete what Social Security needs;

  14. Other duties as assigned.
