HOME HEALTH CARE NAVIGATOR (RN)

Job Summary: Ensure appropriate care plan progression for home health care patients. In collaboration with the in-home care team, confirm the appropriate care plan is developed to address the needs of the patient and subsequently follow that plan progression until discharge. Working with the patient and the in-home team to balance visit schedules and services and prevent unnecessary scheduling conflicts thus improving consistency in staffing. Provide guidance and education to ensure service orders are appropriate for care progression and transitions to the next level of care. Monitor and complete identified care gaps to ensure improved quality outcomes. Education - Required:

• Bachelor's Degree Nursing

  Education - Preferred:

• Master's Degree Nursing

  Experience - Required:

• 3 years Home Health Care Experience

  Experience - Preferred:

• 3 years Home Health Experience with Knowledge of Clinical Record Review

  Knowledge, Skills, and Abilities - Required:

• Ability to set priorities in order to ensure appropriate care plan progression

• Strong working knowledge of the Home Health OASIS tool

• Critical thinking, problem solving and decision making skills

• Demonstrates ability to learn and utilize computer technology to efficiently perform job

  Knowledge, Skills, and Abilities - Preferred:

• Proficient in the navigation of the EPIC Dorothy Module.

  Licensure - Required:

• Registered Nurse Current / Active Pennsylvania Nursing License Upon Hire
