REGISTERED NURSE (RN) HOME CARE ON CALL WEEKENDS/CASUAL WEEKDAY (PART TIME)

Job Summary: Provides clinical support to the Home Care Clinical Services team by assisting in the coordination and delivery of patient care services after normal business hours. Provides direct patient care delivery as indicated. Performs data/order entry, clinical record review and other clinical tasks required for the provision of patient care. Reports to the Patient Care Manager. Minimum Requirements: Graduate of an NLN approved/accredited nursing program required. Minimum of (1) year acute care nursing experience required. Must be flexible to meet patient care needs and travel to patient care assignments over a multi-county area. Must have access to functioning automobile. Maintain and possess a current valid drivers and auto insurance in the state of PA. Must be proficient in English. Basic computer skills required. Ability to multitask. Demonstrates ability to work with multiple interruptions and adapt to changing priorities. Ability to perform duties with accuracy and attention to detail. Demonstrates team behaviors and work independently. Demonstrates excellent organization, interpersonal, problem solving, critical thinking skills required. Demonstrates excellent customer service skills. Demonstrates excellent written and verbal communication skills. Preferred Qualifications: Bachelor's Degree in Nursing desired. Home Health and/or Hospice experience desired. Licensure and Certifications: Current PA State RN licensure. BCLS Healthcare provider certification. Current drivers license;Current BLS Health Care Provider Certification;PA RN Licensure
