MEDICAL SOCIAL WORKER (MSW) PER DIEM - WEEKENDS
Job Summary: Identifies and removes psychosocial barriers to care and assists patients and families to access resources to facilitate their care. Assesses and identifies the psychosocial and emotional needs of patients/families including physical and mental needs, as well as the availability of family and financial support and plans for interventions.
Knowledge, Skills, and Abilities - Required:
Ability to recognize the role of cultural, social, economic, and behavioral factors in the accessibility, availability, and delivery of public health services.
Ability to adjust course of action and plans in relation to others' actions.
Knowledge of community resources and self-help organizations.
Strong assessment and documentation skills.
Requirements: Licensed Clinical Social Worker Upon Hire. Master’s Degree in Social Worker and one (1) year experience in a medical or clinical setting required. Clinical training, hospital or ambulatory training and three (3) years’ experience in a medical setting preferred.
