Position Overview: As an Outreach Worker, you will play a crucial role in supporting students and families through truancy intervention *and *case management. Your work will involve close collaboration with school personnel, community resources, and families to address absenteeism and promote student success. When conducting face to face appointments the Outreach Worker will meet with students and families in their homes or in the schools of the students. These appointments are scheduled based on the availability of families with whom the Case Manager is working, the times of the appointments will vary but are typically during the day into the late afternoon / early evening.
Key Responsibilities:
1. Assessment and Intervention:
- Conduct assessments to address absenteeism and truancy issues.
- Develop and implement attendance intervention plans.
- Engage families in the intervention process and advocate for effective communication between students, families, and schools.
- Facilitate access to community services and support.
2. Collaboration and Liaison:
- Work with school staff, Office of Children and Youth Services (OCYS), and community resources.
- Participate in educational planning and attend attendance team meetings.
- Serve as a liaison between the Agency, schools, OCYS, and community resources.
3. Record Keeping and Administration:
- Maintain accurate records and documentation.
- Timely recording of relevant student contacts and outcomes.
4. Outcomes and Communication:
- Administer and submit outcomes data according to program protocols.
- Keep your program supervisor informed of activities and developments.
5. Transportation and On-Call Duties:
- Occasionally provide transportation for students and families.

Requirements:
- Bachelor's degree in social work or a related field.
- Ability to work *in clients' homes *and engage with the community.
- Experience in family support services and knowledge of family dynamics and child development.
- Strong communication, writing, and organizational skills.
- Reliable vehicle with up-to-date Pennsylvania documentation and liability insurance.
- Adherence to National Association of Social Workers (NASW) Code of Ethics.
- Sensitivity to cultural and socioeconomic differences.
