COUNTY OF STANLY Social Worker II in ALBEMARLE, North Carolina

An employee in this classification provides social work services to eligible families with children under the age of five (5) with identified risk factors with end goals of increased child and family functionality, stability, and self-reliance and/or continuity of care. Work includes interviewing to determine the scope of the problem, evaluating the situation through a needs assessment or other similar tools and identifying appropriate services and programs, developing a service plan, and monitoring service delivery. Work also includes referring clients to a variety of community services to meet financial, psychological, emergency care, emotional, nutritional, and other needs. Driving may be required for home visits or to accompany adults or children on appointments or services. Considerable public contact is involved in working with clients from diverse socio and economic backgrounds. Work is circumscribed by a variety of state program guidelines, health plan guidelines, and procedures. Employees must maintain knowledge and remain up to date on rules and guidelines and related information technology software utilized to access and verify information and maintain case data and details case management notes. Starting pay is $46,460 or DOQ

Examples of Job Duties

Interviews clients and family to determine the scope of the problems and client's ability to deal with it, performing assessments to identify needs and obtaining medical, financial, and social history. Develops client and family centered care and treatment plans and goals related to the identified risk factors, coordinating services to promote continuity of care. Serves as client advocate in the acquisition of needed care and resources, facilitating communication between the client and the provider, and explaining physician findings. Provide client education, referrals, and follow-up based upon need, risk, and conditions. Assist clients in applying for potential financial, health, and other resources, obtaining transportation, scheduling appointments, and understanding medical findings and instructions. Prepares case summaries and reports for reviews, maintaining records, files, and confidentiality. Discusses gaps in services and ways and means to fill gaps with health and other program coordinators and supervisor. Collaborates with health plans, medical providers, and community providers to ensure the program goals are being met. Conducts the required number of contacts each month according to specific program requirements to assess the current situation and assure that services are being provided/received. Explores alternative community care, programs, services, and agencies; makes referrals; coordinates services with other community providers. Provides supportive counseling and education to clients, care givers, and families. Provides a wide variety of services to alleviate crisis situations, enable families to be self-sufficient. Drives to homes, schools, health facilities, and other service agencies. May transport clients. Operates computer, scanner, calculator, and other standard office equipment incident to maintaining records. Participates in patient care conferences. Participates in ongoing training and program audits as required. Ensures all federal, state, and local policies and procedures are followed.

Knowledge, Skills, and Abilities

Considerable knowledge of social work principles, techniques, and practices and their application to specific public health casework, group work, and community problems. Knowledge in maternal, postpartum, child development, medical conditions, and disorders. Knowledge of psychosocial, socioeconomic, and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations, and policies which govern the program. Working knowledge of standard office pr cedures, practices, and equipment. Skill to use a variety of office machines, computers, applicable software and typing. Skill in obtaining health history and patient assessments sufficient to determine if there is any deviation from normal. Skill in collaborative conflict resolution, teamwork and customer service excellence. Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem. Ability to express ideas clearly and concisely. Ability to plan and execute work. Ability to establish and follow work procedures, prioritize work, and prepare and maintain accurate records and reports. Ability to communicate effectively orally and in writing with people of varied social, economic, cultural, and educational backgrounds. Ability to maintain composure, keeping emotions in check, even in difficult situations; and to attend work regularly. Ability to establish and maintain effective working relationships with administrative supervisors, clients and their families, and with care providers and various community organizations

Qualifications

Requires a bachelor's degree in social work from an accredited school of social work and no work experience; or a bachelor's degree in a human services field from an accredited college or university and one year of directly related experience; or a bachelor's degree from an accredited college or university and two years of directly related experience. An equivalent combination of education and experience may be considered. Must have a valid North Carolina driver's license
