Full Time Bereavement Social Worker - Hospice Albany

The Bereavement MSW Counselor is responsible for working with other Bereavement staff to coordinate bereavement services and to provide direct counseling services to individuals and groups. Bereavement staff connect with the primary care givers of patients on The Community Hospice program and connect with them for 13 months post death through mailings, phone calls, memorial services and direct counseling services.

Essential Duties and Responsibilities include:

  • Works with Hospice team members and a team of Bereavement Dept. clinicians to ensure a continuum of care for Hospice families

  • Utilizes clinical skills to assess client needs; to provide ongoing support and psycho-education; and to meet the unique needs of clients who are coping with grief and loss

  • Participates in innovative programs including Wave Riders and Camp Erin Albany; programs that support grieving children, teens and families

  • Receives individual and peer supervision

Minimum Qualifications:

    • Master’s Degree in Social Work, Counseling, or related field from an accredited college or university is required.
  • LMSW required

    • A minimum of one- year experience in bereavement counseling, support group facilitation, or related activities.
    • Three years related experience preferred.
