The Clinical Social Worker provides psychosocial assessment, counseling, crisis intervention, and community resource assistance on a consultation basis to patients and families utilizing a variety of treatment modalities, including individual, family, group and specialized counseling; works collaboratively with the multidisciplinary team to effect positive patient and family outcomes.

Responsibilities:

  • Conducts psychosocial assessment interviews with patients and families to identify problems requiring social work interventions and treatment plans for appropriate services.

  • Provides counseling to impact problems related to environmental needs, family or interpersonal conflict, grief issues, financial concerns, and coping with physical and/or mental illness using a variety of treatment modalities, including individual, family, group or specialized counseling.

  • Intervenes in crisis situations.

  • Assists patients and their families in adjusting to episodes of acute and chronic illness; coping with lifestyle changes precipitated by illness or injury and addresses issues of terminality.

  • Provides consultation to health care professionals when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes; receives referrals for complex problem resolution from physicians and/or care team.

  • Identifies community resources and provides information and referrals to same.

  • Updates and maintains professional knowledge, resource information, skills and competencies related to areas of practice and assigned caseload duties by participation in department staff development and other professional education programs and by development of annual professional goals and activities to meet these goals.

  • Documents social work assessments, interventions, and psychosocial treatment plans according to department and medical center guidelines; compiles statistical reports, prepares specialty documents and correspondence as required.

  • Participates in health system and departmental Performance Improvement Initiatives to effect positive patient and family outcomes.

  • Adheres to NASW Code of Ethics.

  • Completes mandatory education and training in order to maintain organization and department specific competencies and requirements.

  • Maintain s applicable certification/licensure.

  • Other duties as assigned

Education:

  • Master's degree in social work with a casework concentration from a CSWE accredited program.

Certifications:

  • Ohio license appropriate to assigned caseload and practice area.

Complexity of Work:

  • Maintains competencies specific to patient population served.

  • Ability to work collaboratively with patients families multidisciplinary team and community agencies. Strong interpersonal skills.

Work Experience:

  • One to two years direct practice experience.
