CAREGIVER (FORMERLY DAYBREAK COMMUNITY SERVICES INC) Community Facilitator (Abilene) in Abilene, Texas

Position Summary: The Community Facilitator will assist the Day Habilitation Director and/or Day Habilitation Activity Director with individual care at the Day Habilitation based on the policies, procedures, and quality of service required by Daybreak.

Essential Duties and Responsibilities:

  • Directs personal assistance with daily activities of individuals (grooming, eating, and personal hygiene
  • Reinforces counseling and therapy activities as directed by Supervisor
  • Insures health and safety of all individuals by identifying and correcting safety hazards
  • Provides assistance with medications and performs tasks delegated by a Registered Nurse
  • Documents incidents involving individuals at the Day Hab
  • Provides daily reports to Supervisor outlining accomplishments and objectives met on a daily basis
  • Creates and maintains an atmosphere conducive to the habilitation of the individuals served
  • Responsible for cleanliness of day habilitation including cleaning and sanitizing bathrooms, sitting rooms and kitchen
  • Maintains regular attendance and timeliness in duties
  • Other duties as assigned including working in group homes. Must be able to perform all of the duties required of a Residential Support Team Member

Qualifications:

  • Good communication skills
  • Knowledge of the HCS program
  • Ability to handle multiple tasks efficiently and effectively

Education and/or Experience:

  • High School Diploma or equivalent
  • Valid Texas Driver's license
  • Minimum of 1 year working with individuals with developmental disabilities

Language Skills: Ability to read and comprehend instructions, manuals and other company documents. Exhibits average oral communications skills. Position requires good phone, business writing and customer service skills.

Mathematical Skills: Ability to understand basic concepts of addition, subtraction, multiplication and division.

Reasoning Skills: Ability to apply common sense understanding to carry out detailed written or oral communication or instructions. Ability to deal with problems involving unconventional variables in unusual situations.

Duties of this job are performing the following:

  • Sitting is 20% of work time
  • Standing 50% of work time
  • Walking is 70% of work time
  • Bending, pushing, pulling, stooping, reaching, and kneeling is 60% of work time
  • Grasping and finger/hand manipulation is 40% of work time
  • Ability to lift up to 50 pounds
  • Ability to operate computer, facsimile, copier, shredder and calculator

A dayhab setting, varying degrees of background noise, standard lighting, and occasional temperature fluctuations are found in the work environment.
