Avera is looking for a RN –Case Manager to join our Team!

Job Summary

• The case manager is accountable for collaborating with the Multidisciplinary Care Team to promote quality patient care. Facilitates efficient utilization of resources.

• Identifies patient care needs throughout the continuum of care.

• In coordination of care through collaborative practice – has excellent communication skills.

• Knowledgeable of the Health Care Continuum and today’s challenge of change.

• Staff is required to maintain a level of physical/mental health needed to perform their duties effectively.

Education and/or Experience

• AS RN Associates Degree required

Preferred Education

• Bachelors Degree

Required Experience

• 1-2 Years

Preferred Experience

• 3-5 Years

Licensure/Certification/Registration

• Required License/Certification/Registration

• Graduate from an accredited school of nursing with current Licensure as a Registered Nurse in the State of South Dakota.

• BLS

Preferred License/Certification/Registration

• Bachelor of Science Degree preferred, not required

Work Schedule

• Monday – Friday, 8:00 am-4:30 pm; 80 hours every 2 weeks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

• $29.50 - $44.25 per hour
